Anxiety and depression associated with eating disorders and functional dyspepsia: dietary management

Resumo

Ansiedade e depressão são comorbidades frequentes em transtornos alimentares (TA) — como anorexia nervosa, bulimia nervosa e transtorno de compulsão alimentar — e em dispepsias, especialmente a dispepsia funcional (DF). A presença dessas condições agrava sintomas, aumenta custos de cuidado e piora desfechos, inclusive mortalidade nos TA. Uma síntese recente sugere que 55–95% das pessoas com TA terão algum transtorno psiquiátrico ao longo da vida, e diretrizes atualizadas reforçam psicoterapia específica como eixo central do cuidado. Em DF, revisões sistemáticas mostram prevalências elevadas de ansiedade e depressão, particularmente em casos refratários, além de relações bidirecionais mediadas pelo eixo intestino–cérebro–microbiota. Esse quadro clínico impõe desafios ao manejo dietético: no TA, a reabilitação nutricional deve ocorrer integrada ao tratamento psicoterápico; na DF, evidências apoiam intervenções alimentares direcionadas (p. ex., padrões tipo Mediterrâneo, redução seletiva de gatilhos e, em casos selecionados, estratégias pobres em FODMAPs), sempre ponderando riscos de restrição excessiva em indivíduos vulneráveis a TA. Este artigo integra achados de revisões, meta-análises e diretrizes publicadas de 2020 em diante, e propõe princípios práticos para o manejo dietético interprofissional que minimize sintomas gastrointestinais, reduza sofrimento psíquico e promova adesão segura e sustentada.
